Responsibilities:

  • Conducting thorough evaluations of all new patients.
  • Creating and adjusting personalized treatment plans based on each patient’s needs and progress.
  • Monitoring and documenting patient conditions after each session.
  • Explaining diagnoses and plans of care, ensuring patients fully understand their treatment process.
  • Modifying treatment strategies as necessary to achieve the best possible outcomes.
  • Collaborating with referring physicians to provide regular updates on patient progress and status.
  • Ensuring discharge records are complete, including signed progress notes and final recommendations.
  • Building and maintaining a full caseload, meeting weekly patient visit expectations.
  • Promoting long-term care, including periodic follow-up visits for prevention and wellness.
  • Supporting and collaborating with fellow team members to foster a respectful and professional environment.
