Responsibilities:
- Conducting thorough evaluations of all new patients.
- Creating and adjusting personalized treatment plans based on each patient’s needs and progress.
- Monitoring and documenting patient conditions after each session.
- Explaining diagnoses and plans of care, ensuring patients fully understand their treatment process.
- Modifying treatment strategies as necessary to achieve the best possible outcomes.
- Collaborating with referring physicians to provide regular updates on patient progress and status.
- Ensuring discharge records are complete, including signed progress notes and final recommendations.
- Building and maintaining a full caseload, meeting weekly patient visit expectations.
- Promoting long-term care, including periodic follow-up visits for prevention and wellness.
- Supporting and collaborating with fellow team members to foster a respectful and professional environment.
